There are two specific and unrelated issues behind the erroneous behavior:&lt;/p&gt;

&lt;p&gt;The 'pixel' unit, representing the area of a map pixel, was not propagated to all instances of the source map, being correct in some places but not others.&lt;/p&gt;

A workaround is to use the 'show' tool (which supports all the functionality of imagetool) to display the manipulated image, and then Save (e.g. by pressing 'w', or chosing 'Save' from the 'File' menu.).
