I tried dblatex version 0.3-1 (and 0.2.12-4) on an Ubuntu 10.04.1 LTS. &lt;/p&gt;
&lt;p&gt;Further investigation showed that this issue is only present for a cross reference without a predefined label, towards and anchor which is not directly followed by a section or chapter declaration.&lt;/p&gt;
&lt;p&gt;In attachment you can find a simple docbook xml example file which can be used to reproduce the issue. In this example 2 anchors are set:&lt;br /&gt;
- koekoek : in the middle of a chapter&lt;br /&gt;
- koekoek2 : an anchor directly followed by a chapter declaration&lt;/p&gt;
&lt;p&gt;4 cross references are also present:&lt;/p&gt;
&lt;p&gt;- reference to koekoek without a label (&amp;lt;xref linkend="koekoek"/&amp;gt;) : this one fails with the  [?simpara]  output in PDF&lt;br /&gt;
- reference to koekoek with a static label (&amp;lt;link linkend="koekoek"&amp;gt;A nice label for koekoek&amp;lt;/link&amp;gt;) : this one works fine&lt;br /&gt;
- reference to koekoek2 without a label (&amp;lt;xref linkend="koekoek2"/&amp;gt;) : this one works fine &amp;amp; results in a label with the chapter number of the chapter declaration just after the anchor&lt;br /&gt;
- reference to koekoek with a static label (&amp;lt;link linkend="koekoek2"&amp;gt;A nice label for koekoek2&amp;lt;/link&amp;gt;) : this one works fine&lt;/p&gt;
&lt;p&gt;I've tried the same conversion with the FOP tool in which all four cross references work fine. They use the chapter number and title of the first chapter declaration which precedes the anchor declaration. db2latex to transform the XML to PDF via LaTeX.&lt;/p&gt;
&lt;p&gt;I needed a rotated table, so I added a new element&lt;br /&gt;
called sidewaystable, added it to my customzied DTD,&lt;br /&gt;
added a mapping etc. It works quite well. &lt;/p&gt;
&lt;p&gt;Actually, I think this approach is a bit cumbersome,&lt;br /&gt;
because DocBook overs a "orient" attribute, which can&lt;br /&gt;
handle the orientation of a table. So, I don't know in&lt;br /&gt;
particular, where db2latex calls the&lt;br /&gt;
latex-mappings.xml. If you're able to point me to some&lt;br /&gt;
XSL snipplet in block.mod.xsl, and how to call the&lt;br /&gt;
mapping for the table in portrait mode I could offer a&lt;br /&gt;
feature to use rotated tables with db2latex.&lt;/p&gt;
&lt;p&gt;I'm trying to find this section where and how the&lt;br /&gt;
mapping is called by myself, but you guys know your&lt;br /&gt;
stylesheets better than I do and know how the best&lt;br /&gt;
solution for this subject will be.&lt;/p&gt;

Changing this to cvs.sourceforge.net seems to work.&lt;/p&gt;
&lt;p&gt;2) once able to "cvs login", the checkout still fails,&lt;br /&gt;
