Note, my goal here is to test the default behaviour and compare it to
the docbook-xsl edition of docbook. I know a lot of this can be worked
around using overrides, having done so every time I make a book, but as
I believe requiring such overrides to et good looking books scare users
away, I hope the defaults can be fixed.

Table content 'Span Both' should be higher in the cell.
Untranslated 'Equation 0.1'
Missing xref handlings for qandaentry, note, caution, warning, tip,
important and procedure.
Using different gentext key than docbook-xsl. dblatex uses 'step',
not 'Step' in xref. Also 'tip' instead of 'Tip', 'note' instead of
'Note', 'caution' instead of 'Caution', 'important' instead of
'Important' and 'warning' instead of 'Warning' for the block
titles.
