#440 better/more appropriate indices for HTML output

Here are two things that would improve how HTML output handles indices.

1. It would be nice if the indexterm value could in fact be the hyperlink in the html output. (Currently, the value is either the section or chapter title).
2. Failing that, it would be nice if using a secondary element under indexterm would NOT produce duplicate index links when you set "index.links.to.section" to 1 (so it goes to the specific text spot and not the chapter title or section title).

With more ebooks and online content being used, it doesn't make sense to display the chapter/section title in HTML index. It almost seems redundant, and clutters the index with unnecessary words.
