#457 Add capability to make see and see also text italic
Common usage (Chicago Manual of Style among others) is to make the text "see" and "see also" in an index be in italics. This RFE is to add a parameter or equivalent capability (an attribute set might be better) that would let you do this. I've done this and can provide code if that is useful.
