EXDI - wrong exception for missing username

Log in through EXDI (I used the AdHoc tool) and omit the user name. I got an InclompleteNextCount error (!).
1. The user name should have defaulted
2. The exception should have been an EXDI one

Read from DB socket in separate process

Because the PostgreSQL backend can send asynchronous messages back to the front end, the driver should be constantly reading from the socket. This will have to be done in a sepparate, low priority, thread.
