this form builder written in HTML, CSS, JAVASCRIPT but you need PHP Webserver to run it! it's under GPLv3 license because is as plugin of Yana MVC Framework.my(Yana Framework) MVC framework not published yet.for use, just download it and put it in your webserver folder with name of (FormBuilder) and enjoy it.
this form builder is free and server side of it that make database tables and process forms and show to user is not free!
